These services are comprised of "companion care," "personal
care" or "friendly visitor" services. Non-medical
in-home assistance helps older adults with activities of daily living
("ADLs"). These include shopping, bathing, grooming, dressing,
etc.

Nursing
When an individual requires part-time or full-time attendance
of a licensed nursing professional (RN, LVN/LPN) at home. Nursing
care is available on an hourly, daily, weekly or monthly basis.
Chore Services
Service are comprised of lawn, laundry, housecleaning, home management, meal
preparation, sorting mail, etc.
Private Therapy
CHC provides therapy treatments and services, such as physical,
respiratory, occupational or speech therapy by licensed professionals
in the home setting.

Professional Healthcare Consultation
Care-Planning Assessment
CHC conducts a full assessment of the elder's health and living
situation in their home and prepares a customized plan of care and
a Care-Planning Assessment Report. All concerned family members
are then brought into the discussion to make the best choices. Issues
such as home safety, nutrition, medication compliance, depression,
and legal and financial concerns are addressed in the Assessment
report.
Create a Plan of Care and Action
The toughest part of elder care can often be putting a care plan into action.
We provide active assistance in implementing and coordinating a plan of action
after the Care-Planning Assessment Report is complete.
Counseling and Support
CHC’s services include counseling and advocacy referrals.
Provide Education and Life Enhancing Activities
