Statement
Comprehensive Health Care’s (CHC) mission is to provide the highest holistic
approach to serve the people in our community. By gaining access to needed medical,
social and educational services, individuals will achieve the highest functional
level of independence.

Professional Healthcare Consultation, Support and Advocacy
The Professional Healthcare Consultation service allows family members
to plan ahead for elderly loved ones, or assist the elder person
through an experience or event that is impacting their life or threatening
their independence.
CHC’s geriatric care specialists work with each family to
create the best care plan for them based on their unique needs.
Care Planning and Management
CHC recognizes that every elder care situation is different and
that care management services can be used in a variety of ways,
depending on whether families are at early or later stages in the
care giving process. We offer members the option to choose a combination
of services that best meet their needs. CHC acts as a liaison between
members of the health care team, the insurance carrier and the patient.
It is a collaborative process in providing individualized healthcare
needs, which involves assessment, planning, coordination, monitoring
and evaluation of options and services through good communication,
education and utilization of available resources.
CHC’s goal is a client-centered and strength based approach
and commitment to quality healthcare and innovation.
CHC is committed to assisting people to remain as independent as
possible and to keep them free from institutional care. CHC is dedicated
to involving the patient and the family members in developing their
plan of care.
